TEACH Grant Eligibility Request Form

The TEACH Grant program is a non-need-based grant program for students who are enrolled in an eligible program and who agree to teach in a high-need field, at a low-income elementary or secondary school as a highly qualified teacher, for at least four years within eight years of completing the program for which the TEACH Grant is awarded. The student must sign a service agreement to this effect and complete all required counseling prior to receiving a TEACH Grant.

    Teach Grant Eligibility Criteria:

By requesting a Federal TEACH Grant, I understand that I am responsible for knowing all conditions of eligibility, including but not limited to the following:

    •     I have completed the FAFSA and meet the Federal Student Aid definition of an “eligible student”

    •     I will enroll in coursework that is necessary to begin a career in teaching (may include subject area coursework to meet subject-matter equivalency, as approved through the Credential Office)

    •     I understand that my Cal Poly Pomona GPA must be 3.25 or higher, or that I must have scored above the 75ᵗʰ percentile on a college admission test on file at Cal Poly Pomona.

    •     I understand that my TEACH Grant award will be pro-rated based on enrollment in eligible coursework

    •     I intend to be a highly qualified teacher and to teach in a low-income elementary or secondary school, as defined by the U.S. Department of Education’s “Low-Income School Directory,” https://studentaid.gov/tcli/

    •     For every year that I receive a TEACH Grant, I understand that I must complete the online Agreement to Serve and TEACH Grant Counseling at  https://studentaid.gov/teach-grant-program
