Course Equivalency Request

If you would like to request a course equivalency review, please be prepared to attach the following documents with this request:

1. A copy of the transcript from the institution where the course was taken
2. Catalog course description(s) from the year you took the course(s) you feel may be equivalent 
3. Syllabus for each course 
4. Verification/documentation of field work completion (if applicable)

A separate request form is required for each Cal Poly Pomona course equivalency request you wish to have the department review.
